Colistimethate 4.5MIU – A Potent Antibiotic for Multidrug-Resistant Infections

Colistimethate 4.5MIU is a polymyxin-class antibiotic used in the treatment of severe, life-threatening infections caused by multidrug-resistant (MDR) Gram-negative bacteria. It is particularly effective against carbapenem-resistant pathogens such as Pseudomonas aeruginosa, Acinetobacter baumannii, and Klebsiella pneumoniae. Given its potency, it is often reserved as a last-line antibiotic in intensive care units (ICUs) and critical care settings.

Composition and Mechanism of Action

• Colistimethate Sodium (CMS) 4.5 Million International Units (4.5MIU)

• Mode of Action: Converted into colistin, the active antimicrobial agent, which binds to the lipopolysaccharide (LPS) layer of Gram-negative bacteria.

• Bactericidal Effect: Causes membrane disruption, leading to bacterial cell lysis and death.

Indications and Uses

Colistimethate 4.5MIU is prescribed for:
✔ Hospital-Acquired Pneumonia (HAP) & Ventilator-Associated Pneumonia (VAP)
✔ Bacteremia & Sepsis caused by MDR bacteria
✔ Complicated Urinary Tract Infections (cUTI), including pyelonephritis
✔ Meningitis & CNS Infections caused by resistant Gram-negative organisms
✔ Osteomyelitis & Bone Infections
✔ Intra-Abdominal Infections such as peritonitis
✔ Cystic Fibrosis-Associated Lung Infections (via inhalation therapy)

Dosage and Administration

• Intravenous (IV) Dose: 4.5MIU every 8-12 hours, based on infection severity.

• Nebulized Dose: 4.5MIU inhaled every 12 hours for respiratory infections.

• Renal Dose Adjustments: Required for patients with kidney impairment.
